Please complete the form below fully for all repeat prescriptions and we'll arrange to deliver the prescription directly to your chosen pharmacy. 

Some important points to note:

• Prescription renewal requests require 72 hours advance notice however occasionally delays may occur

• Do not order medications that you do not need

• Before requesting a renewal, please check that your Pharmacy does not have a prescription on file that has not been dispensed

• Do not order medications on behalf of someone else, this is both dangerous and illegal

• It may be necessary to have a consultation with the doctor or nurse before repeat prescriptions are safely prescribed.  If this occurs a member of our team will contact you

• If you have a hospital prescription and require it on a GMS one, your pharmacist may issue a 5-day emergency supply directly to you

• It is your responsibility to follow up with your pharmacy to confirm they have received your prescription, ask for it to be dispensed and arrange collection
